One of the most common misconceptions I hear as a Medicare consultant is the idea that Medicare is completely free. It’s a nice thought—but unfortunately, it’s not entirely true. Let’s break it down and clear up this common Medicare myth.
The Truth About “Free”
Many people assume that because they’ve paid into Medicare through payroll taxes during their working years, they’ll automatically get full coverage “free” or at no cost when they turn 65. While it’s true that you’ve contributed to the system, Medicare does still come with some out-of-pocket expenses.
Let’s take a look at what’s actually “free”—and what isn’t.
What Is “Free”?
Medicare Part A (hospital insurance) is premium-free for most people. If you or your spouse worked and paid Medicare taxes for at least 10 years, you’ll likely qualify for Part A without a monthly premium.
What you Still Pay for
If you’re planning for Medicare enrollment, here’s what you should know about your out-of-pocket costs:
- Medicare Part B (medical insurance) – This has a monthly premium. In 2025, the standard Part B premium is around $174.70/month, and it could be higher depending on your income.
- Deductibles & Coinsurance – Parts A and B both have deductibles, copayments, and coinsurance, which you’ll be responsible for.
- Prescription Drug Coverage (Medicare Part D) – Not included in Original Medicare. You’ll need to enroll in a standalone drug plan, which comes with its own premium.
- Extra Coverage Options – Many people also choose either a Medicare Advantage Plan (Part C) or a Medicare Supplement Plan (Medigap). These provide additional coverage but involve separate costs.
